PYL questions

I got a couple of PYL questions.

So I just finished watching 3pm CST episode of PYL on Buzzr and I saw a contestant hit the Whammy four times in a row...

1) Has that happened before?
2) Are there any more eps of contestants hitting the Whammy four times in a row?

Not sure if it was the first, but I'm aware of it happening at least two further times.
