(oh wait, this was mentioned in the original post - and was it Stein or Kimmel (or whoever it was that replaced Kimmel) that asked the questions in the first round?)

Stein in the first round, Kimmel/Pimental the rest of the way.

The recently-canceled Panel Quiz Attack 25 had a host who did most of the duties you would expect, with the exception of reading questions, which his co-host did.
This seems to be the standard in Japan, both for past and present shows. I personally have only come across two shows that haven't split the duties and one of them was Millionaire.

Legends Of The Hidden Temple had Olmec read the Steps of Knowledge questions. He also explained the Temple Games rules, and guided the team through the final Temple as well. Why the heck was Kirk Fogg even there, anyway?

Olmec wouldn't have been able to give running commentary on the Temple Run from his vantage point.

MTV's Remote Control, the Sing Along with Colin category, he "sung" the question of sorts.

Jeopardy clue crew members although they are separate answer readers, not question readers.

On that criteria, I guess Johnny Gilbert and the few categories he's been "featured in" can be counted as well. I also believe Ken did some clue reads as well during Alex's most recent shows.

I'm not counting stuff like the Clue Crew, I'm referring to a different person, on-stage or off, who always read all the questions.
